What to expect
You are about to begin the wonderful experience of healing! For your first appointment please wear comfortable clothing (t-shirt, shorts or sweats), and exercise shoes to your first appointment.
Forms
To get started, we need some information from you. You may simply arrive 30 minutes early to your first appointment to fill out paperwork OR call our office and we can send the forms to you for completion prior to arrival.
payment
Your copay is due at your visit unless other arrangements are made in advance. We accept Visa, MasterCard, American Express, Discover, Cash, Check and Money Order.
